Health Insurance
Yuzu Acupuncture accepts insurance.
We are currently in network with Moda, Providence and Regence. We also accept Motor Vehicle Insurance claims.
If you have acupuncture coverage in your insurance plan, please verify your benefits before coming in for your appointment. We will do our best to also verify your plan, but insurance can be tricky so let’s work together to make sure you have coverage before you come in.
You are responsible for your co-pay at the time of service and any amounts for which your insurance does not reimburse Yuzu Acupuncture.
If your insurance unexpectedly does not pay for your treatment, (due to a lapse in service, high deductible, or some other reason) you are responsible for any uncovered sessions out-of-pocket. If you plan to bill out-of-network, please expect to pay for the session upfront and a superbill will be sent to you. You can submit the superbill to your insurance and they will reimburse you directly.
Please fill out the form on this page and submit your insurance information at least a week before your appointment.
Thank you!
Questions to ask your insurance to verify acupuncture coverage:
“Do I have out-of-network acupuncture coverage?”
“Do I have a deductible that I need to meet for acupuncture coverage?”
“How many acupuncture sessions does my coverage include? Is there an annual limit?”
“Are there any restrictions on the conditions I can get treated?”
